An advance directive is a set of instructions you give regarding the medical and mental healthcare you want, should you ever lose the ability to make decisions yourself. This is your way to communicate your wishes to your healthcare team and helps to ensure you receive the level of treatment you want, if you can no longer communicate with your physician.
