An advance directive is a set of instructions you give regarding the medical and mental healthcare you want, should you ever lose the ability to make decisions yourself. This is your way to communicate your wishes to your healthcare team and helps to ensure you receive the level of treatment you want, if you can no longer communicate with your physician.
Below is a downloadable PDF guide, created by the North Carolina Division of Medical Assistance, which provides additional guidance in regards to advance directives.
Please visit the North Carolina Secretary of State website to obtain an advance directive form (and other such healthcare related forms), as well as additional information, by following the below link or calling (919) 807-2167.
