As a patient, you have the responsibility to:
- Be respectful of your care team, from the providers and medical assistants to the front desk and billing staff
- Be considerate of others in your language and conduct
- Be in control of your behavior, even if feeling angry or frustrated
- Put away your mobile device for the duration of your clinic visit
- Provide accurate and complete information about your health, medical records, address, telephone number, date of birth, insurance carrier, or any other information which is pertinent to your care with our clinic
- Follow the recommendations of your provider as pertains to your course of treatment.
- Take responsibility for the consequences of refusing care or not following your providers instructions and understand that in these instances, we may need to end our relationship after giving you reasonable notice
- Call and notify our office, if you cannot keep your appointment
- Provide us with a copy of your Advance Directive
- Pay any due balances for services provided, or request a payment plan schedule for remittance of any due balances
