Annual Wellness Exams
As part of your health plan, insurance companies require we see all patients for an Annual Wellness Exam, each year. This exam is a preventive visit that typically includes a review of your medical history, screenings for common health conditions and updates to your vaccinations. It also involves a basic physical exam and checks such as blood pressure, weight and cholesterol levels.
Patients should check with their insurance provider for questions related to specific covered items and/or exclusions, as part of their annual exam.
An Annual Wellness Exam does not include discussions regarding new or chronic diagnoses, medication changes, or referrals to specialists.
Annual Wellness Forms
Our office makes every effort to see our patients in a timely manner for their scheduled appointment time. Therefore, we ask all patients scheduled for an annual wellness exam to please come prepared for their visit, with all required forms submitted to our office at least 48 hours in advance.
