
We want to ensure that all of our patients receive the best care possible. As part of your health plan, insurance companies require we see all patients for an Annual Wellness Exam, each year. This exam is a preventive visit that typically includes a review of your medical history, screenings for common health conditions and updates to your vaccinations. It also involves a basic physical exam and checks such as blood pressure, weight and cholesterol levels.
Please note that labs, such as Hemoglobin A1C, which were recently completed, might not be covered again during your annual visit. Patients are responsible for contacting their insurance provider with questions related to specific covered items and/or exclusions, as part of their annual exam.
An Annual Wellness Exam does not include discussions regarding new or chronic diagnoses, medication changes, or referrals to specialists. Should you need to address these concerns, a separate appointment with your provider to allow for adequate time and attention is required.
Should you elect to combine your Annual Wellness Visit with an office visit for non‑preventive issues, or if your provider determines it’s medically necessary, we will bill both services under a single claim to your insurance. Any applicable copay is due at the time of service and if you have not met your deductible, you will owe that portion after your insurance processes the claim.
Our office makes every effort to see our patients in a timely manner for their scheduled appointment time. Therefore, we ask all new patients, as well as established patients scheduled for an annual wellness exam, to please come prepared for their visit, with all required forms submitted to our office at least 48 hours in advance.
A portal message will be sent to you, reminding you to complete any such required forms. For easy access, all office forms for both new patients and annual wellness exams are located on our website, on the Patient Resources page.
If you have not submitted your forms and prefer to complete them in the office, please arrive at least 30-40 minutes early, to allow adequate time for you to complete any required items.
Any incomplete forms will result in the necessary rescheduling of your appointment.
